VITAL RECORDS PROVIDE PROOF OF IDENTITY |
Many times, proof of identity — in the form of birth certificates, death certificates and U.S. passports — is required for travel, employment or receiving benefits. To help Missouri residents obtain these certificates, the Division of Health Services offers a convenient solution.
Birth and Death Certificates
To obtain a driver’s license, seek claims for insurance and other common identifiers, a certified copy of a birth or death record may be needed. Completed in just a few minutes, in-person applications for birth or death records are accepted from 8:30 a.m. to 4:30 p.m., weekdays (excluding holidays), at the division’s office. Copies of birth certificates are $15 each, and death certificates are $13 (plus $10 for each additional death certificate copy ordered at the same time). The department accepts cash, money orders and checks (with proper identification) for payment.
U.S. Passport Applications
Additionally, traveling may require a current passport. Our staff can assist with the completion of a passport application and can take photographs necessary for inclusion in the application.
When applying for a U.S. passport, all adults and children applicants must be present. Additionally, individuals must:
- Show evidence of U.S. citizenship.
- Present a completed application (travel.state.gov) at the division’s office. Please do not sign the document until meeting with staff.
- Pay all application fees by personal check or money order payable to the U.S. Department of State. Any execution or photography service fees must be paid separately by cash, check payable to St. Charles County Government, or credit card (additional fee applies for credit card purchases).
- Present a recent 2-inch by 2-inch color photograph that meets application requirements. This photo can be taken at the health department for a $10 fee.
An individual does not need to be a St. Charles County resident to take advantage of these services.
